What’s the BBL Smell Everyone’s Talking About?
As the Brazilian Butt Lift (BBL) continues to dominate cosmetic surgery trends — even surpassing breast augmentation in popularity — a new and rather unpleasant side effect has surfaced on TikTok and social media: the “BBL smell.”
What sounds like an internet joke is very real for many patients who have undergone the procedure. It's been described as musty, sour, and even rancid — a lingering odor that’s left many shocked… and a few turning to reversal surgery
What Causes the BBL Smell?
According to Dr. Eric Anderson from Impressions Face + Body in Chicago, the smell stems from several post-surgery complications — the most serious being fat necrosis, a condition where injected fat cells die due to lack of proper blood supply.
“The BBL smell is real,” Dr. Anderson confirms. “It can be caused by rotting fat tissue, poor hygiene, or even fluid trapped under compression garments.”
Other, more mundane causes? Hygiene challenges. With a new, curvier shape, many patients find it harder to clean properly, which leads to
bacterial growth and body odor.

*There are also more routine explanations behind the so-called “BBL smell.” Post-surgery hygiene can become significantly more difficult. Some patients report that their new body shape makes it harder to maintain proper cleanliness—especially when wiping—which can lead to bacterial buildup and, unsurprisingly, unpleasant odors. One TikToker who dated a woman with a BBL described the smell as “an assault on my nose,” while another woman said she had her BBL reversed because she "smelled like a dumpster.”
Even in cases where healing goes smoothly, some patients still notice a persistent scent. According to New York-based plastic surgeon Dr. Douglas Steinbrech, “It’s usually related to the healing process.” Sweat and bodily fluids often get trapped under tight compression garments, mixing with bacteria and clinging to the skin for weeks post-op.
